Patient Education

Patient education is a structured, individualized process that helps young people better understand their condition, develop self-management skills, and become more confident and autonomous in their daily lives. Age-appropriate educational resources are available through the QUALIREHAB digital platform.
The Patient Education Pathway
The program is structured around personal assessments and four tailored core modules, designed to meet each patient's specific needs, expectations and goals.
Initial Assessment
A dedicated session identifies each participant's needs, expectations, barriers, resources, and personal educational goals.
Tailored Educational Modules
Educational modules are selected and adapted according to the participant's condition, age, needs, and goals.
Final Assessment and Synthesis
A final session reviews progress and learning, reinforces key messages, and supports the development of a personalized long-term plan.
Group sessions can also foster peer connection, shared experiences, and mutual support.
