Psychosocial support session

Living with a chronic condition can affect emotional well-being, family life, social relationships, school, and work. QUALIREHAB integrates psychosocial needs into rehabilitation, with support tailored to each participant's individual situation.

A Multidisciplinary Approach

Psychosocial needs are considered throughout the rehabilitation process. When appropriate, psychologists, social workers, and other members of the multidisciplinary team can provide additional assessment, support, and referral.

Psychologists

Supporting emotional well-being, coping strategies, adjustment to chronic illness, and psychological difficulties when identified.

Social Workers

Supporting young people and families with social, educational, professional, and practical challenges related to living with a chronic condition.

The psychosocial intervention is individualized to each patient, addressing three interconnected dimensions of well-being.

Emotional Well-being

Supporting emotional well-being and developing strategies to cope with stress, anxiety, and the challenges of living with a chronic condition.

Family and Social Participation

Supporting family relationships and participation in school, work, social activities, and everyday life.

Quality of Life

Promoting well-being and helping young people translate gains in physical health, confidence, and autonomy into meaningful improvements in everyday life.

How Support Is Delivered

Psychosocial support is tailored to individual needs and integrated with the other components of the QUALIREHAB program.

Needs-Based Support

Psychosocial needs are identified during the program, allowing support to be adapted to each participant and family.

Flexible Delivery

When needed, support can be provided in person or remotely, depending on the program, healthcare setting, and participant's needs.

Coordination and Referral

When additional support is required, the QUALIREHAB team can coordinate with appropriate mental health, social care, or other specialized professionals.
